How the application works
- 1. Apply through your local Department of Social Services (LDSS) or NY State of Health
- 2. Functional / level-of-care assessment
- 3. Financial review (income, resources, spousal protections)
- 4. Enroll in an MLTC plan or arrange CDPAP
- 5. Set up services and (for CDPAP) your consumer-directed assistant
Before you file
- Rules change annually — always confirm with New York State Department of Health — Medicaid.
- Transfers of assets in the look-back period can create a penalty period. That is a question for an elder law attorney, not a website.
- You can apply directly. You never have to pay a service to file for you.
